This Certifies that on the 13th day of December 1821, the Presbyterian
ety of Worthington Franklin Co St of Ohio, convened according to
legal notice, & after appointing a Chairman & Clerk protem, Job
Case, Arius Kilbourn & Edward Mallery were chosen trustees, Daniel
Upson treasurer & Samuel Baldwin Clerk & all duly qualified

chosen Trustees Daniel Upson Treasurer & Saml Baldwin Clerk

Nathaniel Little Justice of the Peace and made solom
oath to perform the duties of the several officers to which
they had been respectively Elected according to the best of
their abilities.

   We the subscribers, hereby agree to associate ourselves, together
as a religious society under such rules & regulations, & to
elect such officers as shall be necessary, in order to
constitute us a body corporate agreeable to our act of the
legislature of this State Passed 5th Feb 1819.
